In a way, I think the form he has when he fought Ulquiorra was similar to an arrancar's sword release, hence this is his vasto lorde form, also the ONLY one to be shown. (In the training hole, he simply didn't fully transform, so it was not a different stage. same when he fought byakuya. His vasto lorde form is shown here, and against the vizard. )

@AnimaniacFox No, Ichigos sword is always released. In the early episodes it was still really long but it was double edged and had a guard. After regaining his soul reaper powers with the help of Kisuke Urahara, Ichigos sword reformed (after Cpt Kuchiki broke it) and permanently stayed in its shikai form.
