Majora's Mask was all in Link's head. It's said in OoT that humans (link included) who enter the forest turn into skull kids. Remember the instrument playing guys hanging out there? Humans who wandered in. MM is a mental battle where link fights off the transformation by fighting, well...a skull kid.
