You have your arms through the shirt, and the neck part goes behind the neck, hidden by the over-shirt... and when you turn around you allow the neck part to go through the shirt, and thus the shirt is on, underneath the other shirt :)

This is very simple. He's always wearing the shirt. As he turns his head away from the camera, he only bends his neck & head. He is wearing the shirt around both his arms, so he only takes both hands pulls the rest of shirt up over his head. The shirt is merely rolled up behind his larger shirt and has both arms already through BOTH shirts.
