it's a hookah (Middle Eastern smoking pipe).

Basically you smoke tobacco but it can be flavoured or just good old tobacco. Water is used as filter.

@persipina2008 Iranian all are not persian. Iran is the collection of various ethnical groups that don't consider themselves as persian, sometimes not iranian. Iranian ≠ persian.
