I also had problems filling out the form but then instead of writing 16/7 (16-the number of my building and 7 the number of my flat) I just wrote 16 and it worked out. I hope this helps some of you having trouble whit this.

I was thinking the same thing as someone else. But they do ask for your street as well as your city, and I doubt there's the same street and street number in another city in the same country.. So I guess that's how they do it. And I'm sure they'd ask for your detailed shipping address if you do win just in case you made any errors.
