I'm a huge Lynch fan and I can honestly say that this is one of the most terrifying scenes that he ever filmed. I nearly shat myself when I saw this. This movie is great. There is something deeply disturbing about this film.
The best I've seen from him along with 'mulholland drive' (though mulholland has a slight edge). A tad too long, but truly effective in its attempt to create discomfort and uncertainty.

Lynch doesn't use anything abnormal, like a creature or a deformed face or stuff like that. it's just lighting, and a facial expression. the film has some of the most creepy uses of lighting I've ever seen in a film, it's really what keeps the terror going and keeps you on the edge of your seat
The wonderful thing about Lynch's movies is that you truly cannot predict what might happen next. Take for instance the scene when Nikki is shooting the romance scene with Billy, and then suddenly, right out of the blue, the soundtrack turns sinister and we have the very unsettling camera angle.

At this point in the movie, I seriously considered not finishing watching it. I was watching it alone in my room around midnight, and started tripping balls, as it were.
I did end up finishing it, but then I needed to see The Pick of Destiny afterward to numb the pain.

I love Lynch, but this film is terrible, there's no explanation for this scene, it's free, just to be scary that's all... I love Mulholland Drive and Blue Velvet, but Inland Empire and The Lost Highway I wanto to forget.
He wrote the movie scene by scene practically on a whim, the story is very colloquial with Lynch's mind, severly distorted and mixed around, but there is still a message in there. This scene seems to introduce us to the evil Nikki that she confronts in the end of the movie. The clown face in the beginning makes me think that the phantom is trying to scare Nikki so she won't attempt to free the lost girl and boy who are trapped in Inland Empire.
Trying to explain why this simple scene is scary does require a lot of words and insights into the human psyche. But seeing it, it is very unambiguous in its impact. Genius.

Watched this alone at night with surround speakers. I'm usually good detecting screamers, but this caught me off guard.
But it's really frigging clever. Just as you notice something's up with her face, she speeds into a horrifying close-up.

His films are highly subjective, necessarily, since their subjects are highly subjective.
