It's just an example of Troma's anti-tobacco policies (Lloyd is very much anti-smoking, and this scene probably sealed the deal for him). The tobacco man was a big user of tobacco, and he's talking about himself when he talks about the big-city doctor cutting off half your face (that is, he got his face cut off, not that he's a doctor). Bitter about his own situation, the Tobacco Man is trying to make addicts out of everyone, so they can feel the pain he does. Or so I think, I could be wrong.

Troma didn't make this movie, but they did release it!

The idea is kind of that everything he says is foreshdowing of what is to come, or as much as I can gather from the new DVD commentary!) but I always figured it was a weird, interesting character that could never have a releasable film about him, but could be into RZ. Afterall, if you got Redneck Zombies in your film, you got a sellable film
