Some are small like clarinets, others massive like those Riccola commercial wood horns. Essentially they take native wood, which there is often swirly and tangled/knotted, and they hollow it out and cut a mouthpiece at the end and learn to play it like a trumpet. They're fun but you can pass out from losing so much air so fast on big ones :P
