@gmankillsu he is in pain b/c when a headcrab latches onto a host it drives it beak into the control center of the brain, taking control of the host body, afterwards mutations occur, the arms become elongated & form into branch-like claws & the hosts chest forms a mouth so food can be placed into the stomach, the entirety of this puts the host into constant agonizing pain b/c the host is still alive & aware of wat is happening to him thats he is screaming

This is the "Scientific Explanation":

The headcrab jumps on your head and taps into the motor cortex. At this point, the human is gully concious, but has no control over their actions, but can move their vocla cords. The reason the sounds are low and reversed is because the headcrab is tapped into the brain, which messes up the speach pattern. Personally, I'd try to talk in reverse to get someone to kill me very, very fast.

they probably had a very talented voice actor who was good at making loud muffled screams of pain, but then valve realized that the clips (which by the use of actual words being formed by the zombies give away that the human host for the headcrab can feel all the pain he is constantly in, and then the occasional mutilation / incineration of his body caused by the player) make the zombies overly disturbing even by valve standards, so valve reversed the clips to make the zombies seem less human.
