They were a part of an ongoing Enclave experiment in conjunction with Vault-Tec to test human reactions to off-world space exploration. Each experiment was designed to replicate a possible problem aboard an enclosed environment like a large colony ship.
If there were shelters outside of the US, we don't know about them.

Africa is rich in resources so it was probably plundered and devastated long before the doomsday war. Japan probably got nuked by China. The countries in South America probably nuked eachother over local conflicts too. As for Australia, they probably got nuked by New Zealand, lol.

@thelonewander2277 Just remember the problems that it would cause. For example: A few players could start killing off vital NPCs and irradiating the environments with fat man spamming. Alternatively, the servers could reset every few ours but there will still be trolls who will continuously camp the npc spawns just to stop ppl from completing quests and any new players who happen to attempt to finish the quest will be mown down by the epic troll with a gatling laser. Still, good idea.
@thelonewander2277 True but after a while, the game would be unplayable for new players after all of the "hardcore" players kill them off continuously. Perhaps in a format that had a non-pvp setting it would be fair but meh, it's still a bit iffy. I think that Fallout is a game that is meant to be played single player.

No one is very sure of who shot at who, aside from nukes hitting America. Even Fallout 2 makes it clear that the Great War was a chaotic mess.

The US and China attacked each other (they were in a War, afterall, see: Operation Anchorage). It is possible, however, that other nations fired off their nukes, but we have no evidence of that.
Fallout 2's President Richardson claims that the US nuked China in retaliation. Some logs at Black Mountain also notes that the US fired back at China, but no one else is mentioned.

@DevilTakeMe IIRC the original Fallout mentioned that the European Union broke up and the member nations were at war with eachother, so they probably nuked one another when the U.S. and China launched. Africa is rich in resources so it was probably plundered and devastated long before the doomsday war. Japan probably got nuked by China. The countries in South America probably nuked eachother over local conflicts too. As for Australia, they probably got nuked by New Zealand, lol.

@dark7element Yeah, the Resource Wars leading up to the "Great War" pretty much describe how the world was rapidly breaking down. The European Commonwealth and the Middle East had nuked each other. Then the Middle East Oil Wells dried up, which took them out of the picture. The European Commonwealth then dissolved into a civil War.

@thelonewander2277 Just remember the problems that it would cause. For example: A few players could start killing off vital NPCs and irradiating the environments with fat man spamming. Alternatively, the servers could reset every few ours but there will still be trolls who will continuously camp the npc spawns just to stop ppl from completing quests and any new players who happen to attempt to finish the quest will be mown down by the epic troll with a gatling laser. Still, good idea.

Perhaps in a format that had a non-pvp setting it would be fair but meh, it's still a bit iffy. I think that Fallout is a game that is meant to be played single player.
