a 4 seam fastball is not supposed to have that much tail to it, it is meant to be thrown with maximum velocity. having said that, the middle finger and index finger should be a pencil width apart crossing the 2 seams. the thumb should always be directly under the ball. the 4 seam fastball got its name not because your fingers are crossing 2 seams but when you throw the pitch, the rotation should be tight and when seen in slow motion, you see all four seams spinning vertically.

Your forkball grip is a little off. With the forkball, you wanna jam the ball farther in between your fingers than a splitter, and your fingers you be near the middle, not the top. Also, contrary to what you said in the video, you shouldn't throw all breaking pitches over the top. You can't throw a decent slider over the top. You need more of a low 3/4 angle. You should've mentioned the box changeup, too.
