There's a compilation cassette for the Spectrum called Don't Buy This which was deliberatley filled with terrible games. In one of them it's possible to die instantly because the obstacles can spawn anywhere including the same place as the starting point.

On Kuru Kuru Kururin you can kill yourself in less than a second (though the game gives a 3-second penalty whenever you hit the wall) on most levels! You then go back to the map screen, and can try again as many times as you like, so there's no proper Game Over.

What about Police Quest?

Just type F3 and enter immediately as the game restarts. The F3 key repeats the last command typed and that command is "take off clothes". Not including the game over message, it should be the fastest death. I think the F3 command carries over when loading another file or restarting the game.

Kuru Kuru Kururin (GBA). You can fail almost every level in less than a second! Simply hold A+B and run into one of the walls.

Don't use the game timer though, as you get a 3-second penalty every time you touch a well.

I've played every paper mario and the only time you get a dialogue option that gives you game over (that I can think of at least.) is at the very end of Paper Mario The Thousand year door when the Shadow asks you to join her. sort of an alternate ending of sorts.
