have you tried SVamp quints? Karthus Q gets full vamp if it hits only 1 target(tested it myself, full Svamp and full Rylai's slow on single target). Gives him some nice sustain in the jungle if you can kite small golems.

I use 9/9/12 with magic pen spell vamp also buff prolonging

red flat ad

yellow armor

Blue flat ap

Quint spell vamp

sad the jungle is so nerfed even karth can jungle, the only thing is I dont see him being good with ganks, I mean he has his wall but thats like the worst slow in the game, I can run through it so easily and the slow barely fazes me. I mean at lvl 5 wall its a good slow but ehh

No really, it might be interesting. Q applies redbuff, he has an inherent slow with his W, an easy way to land long range Q's(E) and an ult which could be amazing for ganks. Just wondering if he can sustain in his paths and clear em quickly...

Will we see and jungle Heimer in the near future, I've done it with pretty much no runes (none that help in jungle), with 21/0/9, cloth 5 pots, QWQ. The key thing for me is to pretty much leash for my turret.

@Hoon6105 Spell vamp quints are generally very bad and you'd be better off with straight up AP. You're not going to be doing enough damage to make the cost of using Spell vamp quints worth it. Make sense?
