There was a knife, but they changed it as it was deemed to violent. The card he uses in the mob meeting scene is also upside down, hiding the severed head the "Joker" is holding.

Heath's performance was so amazing in this film. The "LOOK AT ME!" scared the shit out of me. It sounded so demonic and downright frightening.
