I worked at OG when this happened and here's the reality - Olive Garden didn't "accidentally" serve alcohol to a child. Up until this incident, bartenders pre-made their drink mixes in bulk so they could have ready on hand to serve. One employee prepared their own spiked jug of orange juice (which you are obviously not aloud to do), and accidentally served theirs instead of the original one. It wasn't just some honest mistake..
