The doc mentions endocrine problem - endocrine generally means pancreas disorder (juvenile diabetes) and/or hypothyroidism (thyroid doesn't work to regulate weight & requires synthetic hormone just as injected insulin is a synthetic/human/animal based insulin the pancreas should make but isn't in the case of diabetes (type I or type II) though type II diabetes is controllable when caught early w/diet. Juvenile diabetes is ONLY controlled by insulin injections.

My DH was 10lbs at birth but his pancreas worked a little until he was 3-4yo when it quit & he went on insulin. He was a big baby even as 1yo but after insulin injections, he slimmed down & ultimately b/c muscular & almost 6ft tall. I feel for the child b/c he will likely be on life-SAVING drugs for the rest of his life & complications b/c of his conditions. It's sad.
