This is also partly Paulie's fault for two reasons, one is that he doesn't get along with Ralph and two he out-earns him in money. For that Paulie went as so far to call Johnny from jail and let him know that Ralph made a joke about his wife.

I think Phil did because he was so stubborn. I don't think it was fair to say Johnny Sack brought the family down. He spent his time as boss fighting with Little Carmine and cleaning up the mess that it brought. After the dust finally settled and Johnny made peace with Tony and Little Carmine, he was indicted.
