Well, in the eppisode where Sam is still in the hospital, Melinda says that she knows he looks different but she sees Jims soul in his eyes...therefore she pictures Sam as Jim.

@hearse4me69 what she says is; "I see the same guy as you see. Different hair, nose, eyes, it's not Jim. I just know what I see when I look in tohse eyes and Jim, he's in there somewhere, he just has to be". So no, everyone except us sees Sam/Jim as Sam. :)

I think he looks like Jim to Dehlia and Ned and Eli, maybe? Because he flashed back in one episode to Jim when Dehlia finally realized it was Jim's soul in him.
