The confusion arises from the fact that he rarely carries his zanpakutou around with him, as he generally has no use for it, preferring to carry around his medical packs.
Indeed. Although I sincerely doubt that this would actually be represented in the game for the obvious reason that it would make him a really weird character to use -_-

but you don't need to rlly understand it to have fun. basic controls are simple to learn so be easy in no time.
then again i understand about 75% of it hehe

yeah his sword does heal but he can also take all the injuries that his sword has absorbed and transfer it to an enemy. So its not totally useless.

It would make him pretty great as a tag team character though...well...maybe great maybe not, but it would add a new strategy.

No he doesn't, he's a healer. 4th squad members don't have zanpakutou's, but in exchange they can use their reiatsu to heal.
