It is about the crappy infrastructure as well. Most so called bike lanes ARE in the car door death zone. The answer is to either remove the car parking or move the bike lane to a safer location. Or tell drivers that cyclists can travel in the middle of the lane, encourage them to do so and book any drivers that threaten them for doing so.
I've only cycled in Melbourne a couple of times and outside the peaks; other than the (I think stupid) Helmet law I have to admit I was oblivious of the laws; do you have to use farcilities like that?
I cycle everyday and would never cycle that close to a car door that I could be hit if it opened. Surely, these Melbourne bikers should be cycling on the very outer most edge of the bike lane or in the middle of the normal lane.
What kind of road safety campaigners are that, saying that infrastructure is not to blame? Off course the faulty infrastructure is the main problem. You can't expect cardrivers never to make this mistake, it is only human to err. A cyclepath between riding and parked cars is asking for problems. A dedicated cyclepath, seperated from the road is much safer.
Lawyers do not want to correct the problem, but rather benefit over the back of cyclists and cardrivers. It's sickening.

Sure, motorists should look before opening car doors, but bicyclists can't count on it and need to ride outside the door zone -- also greatly reducing the risk of walk-out and drive-out crashes. Car-overtaking bicycle crashes are very uncommon on urban streets, as long as a bicyclist uses lights at night.
