As "fake" as this looks, I don't know how they did it. Puppet? Animation? The main flaw isn't the effect design but the lighting being mis-matched between shots, the same problem as in the original Terminator when they cut between real Arnold and the puppet of his head.

It wasn't designed to be scrutinized. Most people didn't even have VCRs back then. It was designed to shock and it did that very well. 40 million viewers saw this when it aired.
