I favor the idea that it's about a relationship, however it seems that (despite the singer's history) the narrator is the one trying to cope with a partner with emotional issues. And though the song's not about sex, you can't deny that there is an underlying sense of sexuality.

@RobertStalker01 I think its being in love with someone who doesn't take care of themselves...like a drunk or a drug addict, you love them so you can't leave them but they have no will of there own, in fact they hurt or "bite" themselves...they are a marionette, a puppet, wire through the cheek is a reference to the wires used to control a puppets mouth...and those wires go on the sleeve of the person in control...It seems Justin is singing both the part of the puppet and puppeteer.
