Well this is only my personal view on it but it seems to me that there is a couple and they both cheated on each other.

"I give my gun away when it's loaded" would then mean "I am starting a relationship when I'm already in one."

Hence "If you don't shoot it how am I supposed to hold it?" would mean "If you're not going to end this relationship, how will we cope with the situation/how should I keep it?"

The rest should be pretty straight forward after that.
