  • This was an aquired direct inguinal hernia. I suspect your incisional hernia could be repaired via a trans-abdominal laparoscopic approach. As long as there is no current mesh, I would think the risk of significant injury to the vasculature to the testicle would be low. Check out my lap incisional hernia video.

    I have an incisional one with many adhesions left from two previous procedures (appy and later orchiopexy, which was necessary because adhesions from the appy had ensnared the vas). This presents quite a challenge for repairing the rupture, because of all the scarring. I am hesitant to have it done, because of the increased risk of accidently cutting off the blood flow to the testicle.
