Matthew 28:19 talks about baptizing the world in the name of the Father, and of the Son, and of the Holy Spirit.
Later, Christians started using the sign of the cross to remind themselves of the Trinity and of Jesus' sacrifice on the cross.
Scripture doesn't tell us to do this, but so what? It's not a teaching or anything. We're not forcing Catholics to do the sign. It is a simple reminder. Scripture does not forbid it.

Martin Luther, at first, did not want to leave the Church. He just saw some bad things that were happening in the Church and wanted to get rid of those bad things (such as corrupt priests).
Over time, he decided that he wanted to leave because he started coming up with his own theology that contradicted the Church. He found that his theology was contradicted by some Scripture, so he simply removed those Scriptures from the Bible. Protestants have been following Luther's canon since.
That is about right. However, Scripture emphasizes the need to confess (1 John 1:9) our sins to an ordained minister with the authority to communicate God's mercy to you (John 20:23).
Also, we must be contrite. The Lord's Prayer (Luke 11:2-4) demonstrates that we must have the desire to flee from all evil. You cannot be forgiven unless you are truly sorry and want to change.

I would like to point out that I know, because it has been made very clear in The Bible, that Jesus Christ selected Apostles with the ability to forgive sins. But like I said, there is no indication that their descendants or successors can forgive them.
God wants us all to go to Heaven to be with Him. Would God really create such confusion? If something were absolutely necessary for forgiveness would it really require that we just "assume" the true meaning? I think that's what Catholics do.

Oh, succession of apostolic authority? That's easy. The apostles passed in their authority by a laying of hands mentioned in Acts 6:6; 1 Tim. 4:14; Acts 1:15-26; Acts 1:20; Acts 1:22; Acts 9:17-19; Acts 13:3; Acts 14:23; Acts 15:22-27; 2 Cor. 1:21-22; Col 1:25; 1 Tim. 3:1; 1 Tim. 5:22; 2 Tim. 1:6 (and I'll stop here because there are a few more).
We see evidence of this authority having been passed in 2 Cor. 5:18; James 5:15-16; and 2 Cor. 2:10.
