If you're really good, you can actually just walk up to Pyramid Head, then backpedal from the Great Knife as he readies it and repeat. It's actually pretty funny, because Pyramid Head stays locked in the behavior and he looks like a total dweeb trying to hit you.

He's a monster made manifest by Jame's desire to punish himself. Whether the PH is formed from Jame's impression of past prison executioners or whether the monster took that form because the history was "imprinted" on the town is not entirely clear. But he is a creature specific to James and doesn't really belong in the context of any other SH story.

Actually you don't need to shoot him at all. Believe me or not, after several minutes, the siren will start and he will walk away. Just evade his sword all the time. The pros is you save your ammos, the cons is it takes longer times to finish the battle and must really be careful to not get hit ;)

Here's a much easier way - as soon as the fight starts, unload 10 hg bullets into PH. Shooting him slows him down. Then run to the opposite corner. STAY PUT. Reload in the menu and just keep firing 10 hg bullets at a time. It'll slow him down so much he will never get a swing in, therefore making it impossible to die. He'll get closer and closer, but will eventually turn around in the mid-40's of hg bullet hits. It's how i did this part of my 10-star run.

@SuperToughPig If you follow him down the stairs (or follow him at all) he turns around with his knife already in the air) as expected....it kills you on the spot, regardless of how much health you have. Not so much a hard battle....but trust me, during my first playthrough....I was afraid.....VERY afraid.
