Haha this happened to me when I decided to play fallout 3 again... I have all of the add-ons except the mothership one... And I got done with all of them except point lookout... So I went ... Everything was normal.. But after I got some of my *spoiler* brain *spoiler* taken out, I went to the Ark and Dove Cathedral... Every non-generic member inside of the church were nothing but floating heads and weapons!
kinda neat, but nothing terribly unusual. Basically the pictures or "textures" that make up her body are simply not loading. The body is there, it's just not showing on the screen. You're probably playing Fallout 3 too long without restarting your console which in turn got (for lack of a better term) exhausted. Overtaxing your console would easilly prevent the game from doing what it's normally supposed to do. I should know, i'm still addicted to the damn game XD.
something like the skin for the outfit didnt load. i dont think there would be anything to make it transparent. or maybye the oufit did load but its infinately small so you cant see it but the hands and head are in the right spots. i knid like all the wierd bugs in fallout, makes it interesting but there was one time it caused me to have to reload from my last save after i spent 2 hours clearing out a metro tunnel because i got stuck in a pile of rubble. :(
Something like this happened with me but it was my own character, his legs were missing so I was floating legless in the air XD Game crashed when I tried fighting some muties in vats XD
Yeah... Sometimes has insane load times, no voices, bad lip-syncing, and so on. Still love it. From what I understand, the PC version doesn't have most of this stuff.
I had a similar glitch with a Merc but just his head showed and for his body was a uneven(not sure on color)triangle. it happened after i tried jumping a wall of tires and when i looked to a certain direction it got all blochy with gray and brown triangles(all there pionts from my reticle).

I did that off camera. Shot her non-existing torso and blood went everywhere. So funny...
