You can eitehr conceive with a male (using the male HUD) or set your pregnancy status manually (without a male). then the "countdown" starts. When the pregnancy nears its end, you can at any time induce the birth yourself, sicne, it is Roleplay. And for Roleplay you need to get your people (otehr Roleplayers) organized. So you can prepare all you want, get your midwife ready, your partner, yor friends, whomever, and when the time is right, you start the birth at the place of your choice.
