I feel the human is a bit easier than a kettlebell, but nowhere as easy as a dumbbell. Problem with the KB is that it puts considerable torque on the wrist - I used a kettlestack loaded with 25lbs plates for training - which doesn't occur with the DB and only slightly with a human. A sandbag held on the flat of an outstretched hand is worst, though - I worked up to a 130lbs bag this way before attempting the girl, just to make sure - not too good to look bad in public...
I don't have any issues with wrist torque, because I keep my wrists straight. The only thing I can think of that would cause one to torque his/her wrist is by having the wrist bent/extended backward towards the forarm when gripping the bell. If you keep your wrists straight and fixed at all times, there should be no torque issues.
You are correct in that the safety of this move is contingent upon both parties knowing what they are doing. That being said I would have a spotter or some matting just in case. Just my opinion.
When I did this feat, I told my partner that I couldn't guarantee it would all go as planned - since there was no warmup etc - but if I'd drop her, I'd drop her onto me, taking the impact. First she hesitated, then she agreed. And we pulled it off no sweat ;-)
she made it a bit harder for you obviously , didnt tighten up enough and left her arms too loose .

I like yor good form, just at the very end before you stand completly you push your leg forward - not so good for joints.
