i think its implied that sarah wears it all the time or at least keeps it somewhere. in s5e7 when shaw comes back, shaw is shown taking it from sarah and chuck takes it and gives it back to sarah afterwards

i think she wore it one time in another episode... i forgot which one though. i think it was when she told chuck that cole was captured and they had to do 24 hour surveillance. when they hugged in that episode, she was wearing that bracelet.
