Actually, i think that you can make a death knight once you reach level 55, thats their plan for now.However it can be changed.

you don't have to be a warlock, im pretty sure all classes can do the quest. And it will most likely be at lvl 80 or between 70 and 80

it has not been released what lvl you have to be to be able to do the quests to unlock the deathknight, but im assuming it will be lvl 80

Actually somone kinda said that you have to be 80 there will be a questline availeble for all classes but i think Death Knight sucks.
