The game keeps track of how many continues you've used for you - check the last digit (or last 2 digits) or your score and that's how many continues you've used :)
Otomedius Excellent is very much a 1-life game. There are times where recovery is extremely difficult because you lose all your weapons and powerups after each death. Fortunately, there is a force field power up (the ? powerup) which will allow you to take 5 hits without losing a life.
BTW You move around WAY too much.

@Korszca Your absolutely right on the FORCE FIELD power up. Playing the game on Easy will give it to you from the start after every death.
