...you will need to gather all medical records from your doctor. from the past.... if the diagnosis code billed falls under your pre existing then the insurance will not cover that .. but if its not , then the claim will be paid... it will be reviewed by medical review and they will be the one to decide... (provider care )

Thats what you call a automatic no. They didnt even look at it. Every third claim is denied and yours just happen to hit on the lucky number.

I was denied payment by BlueCross on a claim for a yearly physical. The reason? Pre-existing condition. I asked them what was the pre-existing condition, the fact that I was alive over the past year? Of course, I got no response to that.
