Yes, he does claim to be "Martin" Van Nostrand. In the episode when they are having auditions for the pilot "Jerry" Kramer enters the room to audition for the role of Kramer (himself) and the receptionist introduces him saying: "This is Martin Van Nostrand, auditioning for the role of Kramer" So he registered for the audition with that name.

At no point in seinfeld does Kramer state that his name is Dr. "Martin" Van Nostrand. He only ever says his name is "Dr. Van Nostrand"
