the little boy is the one narrating the story , lara was killed i think but that was the last time he saw her

I don't know if you watched the whole movie in English, but Lara is being taken away because she was the wife of Pasha. Both her and Zhivago were on the Communists' hit list. The little boy is holding the book of his father, Zhivago's poetry, Lara has most likely been killed in a concentration camp. Hope that helped you out.
