I acually find it a little odd that pump action shotguns are preferred over lever action shotguns today. If you were to fire a lever action shotgun, and miss, you could still hold the gun steady with your left hand (For right handed people) for the next shot, whilst with a pump action shotgun, you would have to move your supporting hand, which would throw you off sight for a moment, though these old lever action shotguns are slow at reloading, newer ones would be much more efficent.

@Grungadin even some realistic games has unrealistic shotgun spread (like OFP2). And what balance is in BC2? Some maps in BC2 are hundreds of meters in size. There are long distance fights (100, 200, 300 m) but shotguns in BC2 are very inaccurate and weak. Balance, yeah.
