Actually word "Kafir" is used for different meanings in Urdu poetry. It is used for "beloved" and for "cruel" or "tyrant" also. Both these meanings are not the linguistic meanings of this word, but these are evolved through the tradition of Urdu poets. Linguistic meaning of kafir is, as you know, the "infidel" or "heathen" (one who does not acknowledge the God).
So here "kafir haseena" means "beloved beauty" and / or "cruel beauty".
