it's just the bottom of his teeth. The part where it touches the gum looks sharp because his teeth are a little untreated, so it has deeper holes in his gum. What looks like the bottom teeth is just his gum from the bottom, and what looks like the top sharp teeth are his bottom teeth touching the top of his mouth.

This is an illusion, but it is symbolic. I wouldn't brush it off completely, but it's not evidence or anything.
