I remember finding this hard to take in upon first seeing it.

Here is this new character who is tougher than the other members yet we don't why at the time. He's sitting in a big tongue in a highly industrialised environment which is totally different to Konoha.

The leader of Akatsuki has nicknamed himself "Pain" because he wishes to act as a god and show the world pain to make it grow up and bring peace.

Pain and Pein are both pronounced "pain," the name itself is based on the English word "pain," and through an English spelling, it's spelled "Pain."

The reason why it's spelled Pein in the manga is because that spelling comes from the Japanese kanji used to spell "pain." "Peh," (Pe); "ee," (i); "n" (n).
