I don't know about the planet's name, but the coordinates are (in order):

Orion - Pegasus - Hydra - Scorpius - Gemini - Aquarius

A lot of them are at a weird angle, and the quality isn't so good, so it's hard to tell, but if you just count clockwise/counter-clockwise from the point of origin symbol, and then compare with a picture of the gate, you can figure out what the symbols are.
