The mast was too high, hit a crossbeam on the racking and catapulted about 30 boxes (approx 200lbs each) through a window and out into the street. Blocked traffic for about 4 hours.

@heathlive If you look at the object he is carrying right before the shelf collapses, the object hits the rack and turns slightly, pinning it behind the rack and the object to his left...forklifts are made to have a lot of torque...so my guess is, the objest turned, got pinned, and he stepped on the gas. Shelving units are made to support from the ground up, not support 10000 pound machines pushing into the side of the base of their support.
@furbyoats In a warehouse I worked in, a turret truck hit the supports on a rack hard enough that it tore all of them out of the concrete for half the length of an aisle, along with big chunks of concrete in some cases. Still, the shelves didn't collapse like in this video. Something was wrong here with the way they were installed.

The cross members are also bolted into place. I have seen Home Depot racking hit much harder and it doesn't even shake, much less collapse. Orange racks aren't automatically HD.
