Are you talking about how there's different people in certain episodes?

If that's your question, Junjou Romantica has three sets of couples it focus's on this one called: Junjou Romantica (Title/main focus), Junjou Egoist (Second Focus), and finally Junjou Terrorist (Last one specified.)

So don't worry about being confused (I was two at first,) once you realize this it's actually pretty easy to understand. You can just skip the episodes of the pairings you don't like ^^;
