At 4:20 she meets the emprisoned women which is like a ghost, and she starts grasping that she is caught up in a ghostly world where life are intermixed between high hollywood mafia and poor polish prostitution. The two worlds have mixed because of a gypsy curse. The film itself has become a ghost trap. So this moment is key: the prostitutes are both the polish and the hollywood ones, the world is universalized. The ghost tells her to look at an underwear while she lives her life to understand.
The red bulb is like a symbolic representation of evil. You see the main evil of the film with a light bulb in his mouth at some point. He's gone to Inland Empire (he's not in poland anymore). There seems to be a crossover between him being some kind of pimp in poland and Sarah's Husband seemingly being a mafia head in the beginning (whatever he is, a bad guy). The world has been reversed as originally he was a good polish guy. It's really about narrative and reversing two worlds.
