It's not so common to order because it's so small...good if you just wanna shot or two of straight espresso. Next size up is called "tall", which is the small size in all others shops.
Basically, Starbucks has it's own size system which is the same in Japan as well as America.

i think "Tennai de" is more common in cafes than "kochira de"... most common i hear is "tennai de omeshi agari deshouka"

is a word for go when you use it for your self. its different from "to go" when you say "for here or to go?" ...difficult language... lol

The book's fine; it's a different context. :o)
Think of the example as meaning `take home' or `take out'.
I'd recommend avoiding literal translations between Japanese and English. They're so different that it just doesn't work!
