It's kernel bug, so I think that none from us can know how this happend. I have just an idea about bug in someone's system (maybe mine - host's). Some data was simply lost (like "relase from target" or something).
I saw Rhasta cathing Ezalor when he was casting Illuminate, Rhasta catched Ezalor, he stopped channeling, died, and killed Rhasta will the Illuminate wave, they both died, and when revived, Rhasta was holding Ezalor with shackles from Sents respawn to Scourges, long shackles to see :)
