I am a Studiomini iPhone and iPad user. Great video and great idea to get multitrack monitoring to work easily on the iPad. I wanted to inquire as to what KV Connection adaptor you are using? Is it the one with the attenuator? It doesn't look like it. I'm currently using a 4 way cable from my video camera to do this and was trying to decide which KV cable to buy. The video camera cable has some unwanted noise.

I'm using the 1/4” line-level audio input & 1/8” adapter cable by KV Connection.

Camcorder AV cables work with iPhones up until the 3GS, but newer devices will pick up noise, unless you use a cable specifically made for iOS devices.
