Romano passed out in the OR because of a kidney stone. He didn't need surgery though. That part is from season 6, Abby Road. Romano wouldn't let Benton do plastic surgery on a girl who had been bitten in the face by a dog. But after Peter stepped in during the surgery when he passed out, Romano owed him a favor and let him operate on the girl anyway.

I think it was when benton and elizabeth operated a person (without romano's licence 'cause benton is just not allowed to operate without romano's licence?? i dont know right..) and romano came into the surgery and he shuted with benton and asked him to leave the room, and he countinued instead of benton but romano started to shake and after some minutes he fell down and paroxysm or attack or i dont know and eliztabeh helped him and maybe he needed an appendectomy maybe,
