The knot goes off to the side of the head, not the back. The idea is to break the neck immediately. The knot towards the back of the neck is slow strangulation. Also the drop must be calculated by the persons weight. Too little a drop wont break the neck and too much drop will pull the persons head clean off.

Ive done alot of research on hanging and this is what I have found:

Generally it takes 10-15 seconds to lose consciousness. After the loss of consciousness the brain slowly starts to die because blood can't pump to the brain so there is a lack of oxygen. It generally takes 30 minutes to die after blacking out. Keep in mind that 30 minutes is keeping it very conservative.
