I will explain - (sigh) most Gay men 'think' they are fat when they really aren't. Gym obsessed men who think anything above 4% body fat is repulsive. To a straight man he would probably be considered thin/slim.

The reason for this was in the 80's people started to contract HIV eventually AIDS and the only way to discern yourself from the sick was to look as healthy/fit as possible, working out etc. That is how the stereotype exists today. Hope that helps
