You go out of tune when you hold a note sometimes. It sounds like you're guessing the note (or just remembering how the song goes) rather than knowing how it sounds before hand. I would practice tuning your vocals by singing the notes along with a piano. You will tighten up those small mistakes very fast but it will be a huge improvement.
